- Your Own Email (if yours isn’t found, try your spouse’s)
- Your Child’s Name
- Child’s Date of Birth (mm/dd/yyyy)
- Child’s Gender
PLEASE NOTE: Regardless of whether you have seen Dr Watkin at previous practices, you will be a NEW PATIENT the first time you see her at Optimal Healthcare Ltd.
